3 Declaration and acknowledgement
I confirm the evidence is accurate and my own. I understand that applying does not guarantee credit. For a challenge, my learning will be assessed against the Ontario curriculum using formal testing/examination (70%) and other assessments (30%). The principal will confirm my pathway and credit limits before approval. I understand that challenge passes, failures, and withdrawals are retained in PLAR records, and Grade 11–12 completed challenge results, including failures, are subject to full disclosure on the OST. Equivalency awards follow separate recording rules. Adolescent challenges allow at most 4 credits, with no more than 2 in one discipline; mature limits differ.
For challenge applicants: [ ] I reviewed curriculum expectations and achievement levels with the designated subject teacher.
